Abstract
Screening questionnaire woth 81 items, includes seven areas of functioning: substance use / abuse, mental health, family relationships, relationships with friends, educational level, employment interest and aggressive behavior / delinquency. Items are presented with dichotomous response option (yes / no)

Way of administration
The administration of this questionnaire can be done individually or in group. The first form of application is used when the user comes personally to request a service and the group administration is used in groups, predominantly at schools where it is desired to detect those adolescents with indicators of problems associated with alcohol or drugs.
Way of qualification
Starting from an affirmative answer to one of the 19 items of the Use / abuse area , it is considered necessary that the adolescent receives assistance as this indicates the presence of problems associated with use.

Schools and clinical practice. It needs to be led by a trained applicator.
